Individual Enrollment Application
This form allows you to open a new individual TNStars account
Automated Contributions Authorization/Change
This form allows you to establish or change automated contributions to your TNStars 529 College Savings Program account from your bank account.
This form enables you to rollover all or a portion of your account balance from another 529 program into a TNStars College Savings 529 Program account.
State Payroll Employee Form
This form allows you to set up a payroll deduction into the plan. Please check the Employer Resources page to ensure that your employer is on the list.
Account Maintenance Form
This form allows you to make an address change, change Purchaser Appointee information, and other functions such as automatic contributions.
Withdrawal Request Form
This form allows you to request &amp;nbsp;a withdrawal from your TNStars College Savings 529 Program account.
